Well, there's only ONE light used in the picture. It's camera left, pointing towards the ceiling. Beside the flash is a "wall" of computer printer paper, like in the shot of the crosshairs above.
What happens with the light is this: the paper catches a lot of the light from the flash, because it's so close. That's what makes the really bright white part of the reflection on the left side of the lenses. The rest of the reflections are from the light that hits the ceiling.
